School Lunches
Medication
The following forms are provided for the use of medication in school. No medication can be issued in school without the appropriate form. Please contact the school if you are unsure which form to use.
Use Form1 for non-prescribed medication to be administered by the school
Form 1 Non-prescribed medication
Use Form 2 for short term prescribed medication to be administered by the school.
Form 2 Short term prescribed medication
Use Form 3 for long term prescibed medication to be administered by the school.
Form 3 Long term prescribed medication
Use Form 4 for medication that will be takeadministered as required, e.g. asthma inhalers
Use Form 5 for medication which should be self-administered
